According to an Associated Press article published by mySA, workers placed a large wooden box on a forklift hoist and two workers then climbed inside. As the box was being moved to the second floor of the construction site, it became unsteady and fell to the ground. One worker managed to jump out into the second floor of the building in enough time to escape injury but the other fell to the ground and was killed after the box fell on top of him, the article said.
