Doctors who pass a periodic assessment of medical knowledge to maintain ABIM board certification have fewer state medical board disciplinary actions

Philadelphia, PA, March 8, 2018 – Doctors who practice internal medicine and pass a knowledge assessment to maintain board certification within 10 years of their initial certification are more than two times less likely to face state medical board disciplinary actions than those who do not pass the exam.

These findings about Maintenance of Certification (MOC), recently published in the Journal of General Internal Medicine, build on previous research that found physicians who pass initial certification exams administered by the American Board of Internal Medicine (ABIM) after medical training are five times less likely to face disciplinary actions than doctors who do not become board certified.

ABIM data indicates that a vast majority of internists pass the certification exam after training and periodic MOC exams through their careers. Even more ultimately pass on subsequent attempts. To explore whether there is an association between MOC exam performance and risk of disciplinary actions from state medical boards, ABIM researchers studied MOC exam results and any reported disciplinary actions for nearly 48,000 general internists who initially certified between 1990 and 2003.

About 2 percent of the population-949 physicians-were disciplined during the study period. Research findings showed that:

The risk of disciplinary action against physicians declines as scores on the MOC exam increase, indicating that more medical knowledge is associated with fewer disciplinary actions.
The percentage of total disciplinary actions in this population that can be attributed to not having passed the Internal Medicine MOC exam is 35 percent.
Poor exam performance is associated with more severe disciplinary actions.
There was no difference in disciplinary rates associated with the amount of Continuing Medical Education (CME) required for state medical licensure, suggesting that completing CME alone in the amounts required for state licensure does not reduce the risk of disciplinary actions.
Researchers estimated that the number of patients potentially cared for by physicians with disciplinary actions could total hundreds of thousands to a few million.

“These findings have significant implications for a vast number of patients seeking safe, quality care from general internists,” said Dr. Furman S. McDonald, Senior Vice President of Academic and Medical Affairs at ABIM and the lead author of the study. “Though most internists will never face disciplinary actions, the study revealed an important association between medical knowledge as demonstrated on the MOC exam and lower risk of disciplinary actions. This adds to the growing body of evidence demonstrating that MOC is an important factor that can inform the public’s choice of physicians.”

Arizona Company Encompass Inspections Featured on Travel Channel

Arizona company Encompass Inspections, known for using multiple technologies for subsurface investigations, recently aided Travel Channel show Expedition Unknown in an underground search with a historic twist.

“We were honored to be part of this project and certainly floored when our team actually found an anomaly right in the location they suspected Butch Cassidy’s body,” said Encompass Inspections President Lance Jehn. “It was great working with the Travel Channel on such an interesting project with such historical significance.”

The show follows host Josh Gates as he investigates the world’s most enduring legends and mysteries. In a recent episode regarding legendary outlaw Butch Cassidy and the location of his grave, Encompass Inspections delivered cutting-edge technology to conduct the search.

In addition to traditional applications in construction, technologies like ground-penetrating radar (GPR) are commonly used by law enforcement to find unmarked graves and for other forensic work. Subsurface search technologies locate, measure and determine depths of items underground, in soil or in concrete.

Chosen for its exceptional search capabilities, Encompass Inspections scanned a large area of a mountainside in Johnnie, Nevada. The mountain’s old mine is where some allege Cassidy was buried 100 years ago. After identifying multiple potential gravesites, the Encompass team narrowed the search to a particularly promising site, which happens to correspond to eyewitness accounts and historic photos.

Fischer Identity 6.1 Controls Business Operations – Not Just Security

March 7, 2018 – Naples, FL – Fischer International Identity unveiled the latest release of their Identity Governance and Administration suite that places heavy emphasis on “Control.” Fischer believes that Identity offers organizations ultimate control over the entirety of their organization, and should not focus solely on security.

“Security happens organically if you deploy the right IGA & Access Management product suite” says Dan Dagnall, COO of Fischer Identity. “We are extremely excited about our 6.1 release, as it creates a synergy across the entire IGA stack by offering the proper mix required to secure an organization, and the necessary functionality and scalability to easily automate IGA across the enterprise. The result is control – control of business processes speed, security, costs, and efficiency.” Dagnall asserts that mature Identity products must provide Granular Security, Streamlined Functionality and Enhanced Governance that are predictable, affordable and right-sized for any organization. “That is exactly what we’ve accomplished with version 6.1.”

Granular Security Enhancements
Fischer now offers a native key management feature, empowering organizations to review keys used for their organization or globally within the product. It provides transparency on what is being encrypted, by which algorithm, and how long that key has been in use. It also empowers Administrators with the ability to rotate a key which will, in effect, create a new key and re-encrypt every existing value with the new key. Also new is manageable key-pairs used for transmitting sensitive data over the wire. The asymmetric encryption for sensitive data provides an extra layer of security and does not interfere with SSL nor does it remove the need for SSL. And for ultimate protection, native key expiry policies have been introduced that will automatically detect stale keys and immediately trigger a rotation if an administrator has forgotten to rotate or if a key has been in use for too long.

Following the NIST standards, Fischer Identity 6.1 introduces new algorithms for encryption. “We are also proud to further align our features to match best practices and recommendations from organizations such as NIST to ensure that our current and future customers are able to rely upon the hardened security best practices that our industry has adopted through the years.” For symmetric encryption (used for data at rest), Fischer defaults to AES-256 and introduced expanded support for AES-192 and AES-128. For asymmetric encryption, RSA 2048 is included by default and adds RSA 1024. The product’s hashing algorithms have also been upgraded to the latest and greatest available in the market.

Streamlined Functionality
Fischer continues to lead the market with a WYSIWYG approach to IGA & Access Management, a competitive differentiator for both Fischer and their customers. “In this release, we’ve expanded our customer’s ability to quickly migrate solutions between environments, literally with the click of a button. We’ve been able to migrate Fischer on-premises deployments to our Identity as a Service® cloud in a matter of a weekend” Dagnall boasts.

Fischer also expanded the User Match capabilities to help organizations capture duplicate identities BEFORE they are created.

6.1 brings with it native abilities to execute automated processes “onSubmit”, without writing a single line of JavaScript code, or XHTML changes to call a custom process. “Post processing is essential and forms-based interfaces often require source code to properly execute a post process. Organizations can now leverage Fischer’s well known point-and-click approach to execute business processes as a result of a user submitting a form. While this may seem like a small change, tell that to those tasked with upgrading our competitors’ solutions. This small evolution in our product offers maximum scalability to our customers.”

Fischer has also expanded support and integration with Duo Security.

Enhanced Governance
Fischer Identity 6.1 brings governance to the forefront by offering more granular options than in previous releases. Recertification jobs can now be executed faster and the attestation process has been improved to include the proper stakeholders, putting Fischer’s latest release of re-certification and attestation on par with the leaders in the market. “With our focus on provisioning and our long history and depth of capabilities relating to automation and fulfilment, refactoring certification to match how the feature has come to be used by most organizations was not difficult for us at all. We feel our investment in provisioning over the years puts us light years ahead of our competition when it comes to assuring the accuracy of the data used for attestation and certification.”

Fischer also expanded their customer’s ability to store useful attributes about each of their unique identity types. “Fischer understands that not every type of user fits into a single identity and it is very important that each organization’s own identity types are able to be properly represented. More importantly, the distinct attributes about the different identities must be available to the Identity solution so they can be controlled within the context the customer’s business needs. For example, Travelling Nurses need access quickly to deliver services to in-home care patients, while Visiting Faculty members need access quickly to sponsored university accounts. These are two entirely different scenarios that demonstrate why your IGA vendor’s data model must be able to grow with your business and provide the right-sized enforcement for each and every different persona your organization must secure and control. 6.1 gets it done for you and is purpose-built to grow with your business.”

Dagnall concluded that “Niche governance vendors were definitely necessary to help our industry point itself towards the future, and that future is one where cyber security, access management, authentication, authorization and provisioning are bundled together where Identity is the ultimate authority. I am excited to report Fischer International Identity is well down this path and we are excited for our next major release which will continue to provide best of breed features, and visionary approaches to solving your Identity, Governance & Access Management needs.”

Additional Platform Support
In addition to Fischer’s support of customer on-premises, private cloud, or Fischer IaaS® environments, Fischer Identity Version 6.1 is now also available for Amazon Web Services environments, combining the industry-leading IGA solution with the scalable and services-rich AWS platform. Support for Amazon Aurora and AWS APIs is included.

About Fischer International Identity
The Fischer International Identity mission is simple: “Your Success.” From how we’ve engineered our identity & access management and governance software, and how our implementation methodology eliminates unnecessary overhead and cost, to our investment in our customers by providing low cost and in many cases no-cost feature enhancements, our solutions make IAM work for your organization – both technically and financially. We never stop innovating. We never stop evolving our product and our methodology. We are never satisfied because we know we can continue to make IAM and IGA easier to consume and deploy.

Sacramento, CA Author Publishes Police Diversity Book

Selecting, Assessing and Evaluating Applicant Cultural Diversity Background in Hiring Police Officers, a new book by William F. Gaines, has been released by RoseDog Books.

As a Retired California Peace Officer it is the author’s sincere hopes and wishes that this particular book can open and create a dialogue on “How Police Officers Are Selected, Assessed and Evaluated” in becoming a police officer in the community.

William F. Gaines feels strongly that this is the right book at the right time. The book is an attempt to develop a broad range of questions in order to develop a testing model that can be utilized to assess whether or not this particular applicant has the life experiences, cultural awareness, social interactions, peer group associations and if this applicant has the temperament suitable in becoming a duly sworn police officer.

The book takes a closer look at how cultural diversity should and could play an important role in selecting, assessing and evaluating those applicants prior to becoming police officers. This is extremely important and very crucial when police officers have contact with those of a different racial/ethnic group, different lifestyle, different environment and very different social conditions.

About the Author
William F. Gaines is a duly sworn Retired California Peace Officer with the California Youth Authority better known today as the California Department of Corrections and Rehabilitation. The California Youth Authority was a Juvenile/Adult Correctional agency that incarcerated both Juvenile and Adult criminal offenders ages sixteen through twenty-five.

Gaines is fortunate and humble as the only one among his siblings to graduate from a California Community College and a California State University. In 1979, he graduated from California State University in Sacramento, California, with a B.A. degree in Sociology and a minor in Psychology. While attending California State University in Sacramento he was able to graduate with honors for achieving and maintaining the dean’s list from 1977 through 1979.

Having worked in both classifications as a California Peace Officer responsible for Custody/Security and Treatment has afforded Gaines invaluable knowledge and field experience in dealing with criminal offenders from all racial groups, all geographic locations in California, all economic groups, all social classes, all street gang affiliations and those offenders deemed untreatable, unmanageable and hard core.

After spending time in the County Foster Home system one thing life has taught Gaines is to always believe in yourself and eventually others will believe in you.

Selecting, Assessing and Evaluating Applicant Cultural Diversity Background in Hiring Police Officers is a 30-page paperback with a retail price of $9.00. The ISBN is 978-1-4809-7484-5. It was published by RoseDog Books of Pittsburgh, Pennsylvania. Louisville Orthopedic Surgeon Offers Stem Cell Injections For Pain Relief

Louisville orthopedic surgeon and sports medicine doctor Stacie Grossfeld MD is now offering stem cell recruitment therapy to patients in her Louisville, Kentucky office. This cutting edge therapy is used to provide dramatic pain relief for many common medical conditions including osteoarthritis, knee pain, shoulder pain, hip pain, tennis elbow, plantar fasciitis and much more.

Stem cell recruitment therapy is part of a broader trend toward orthobiologic medicine. Orthobiologics are substances found naturally in the human body used to help heal injuries and conditions. Other examples of orthobiologics including: hyaluronic acid injections and platelet rich plasma (PRP) injections.

As a double board certified orthopedic surgeon and sports medicine physician, Dr. Grossfeld explains her interest in offering stem cell recruitment therapy to patients: “As an orthopedic surgeon and sports medicine physician, I’m very focused on helping my patients heal as quickly and effectively as possible so that they can get back to the activities they enjoy. I’m always learning about the newest treatments available so that I can offer the best that medicine has to offer. Stem cell injections are the latest example of this … a treatment that uses natural bodily substances to offer pain relief through actual healing, rather than just masking pain and injury.”

Stem cell recruitment therapy is a safe and effective treatment to help the body help itself. It’s typically administered during a normal office visit, through a single injection. Patients have reported improved pain relief and repair for the following conditions:

Joint injuries
Osteoarthritis
Knee pain, shoulder pain, hip pain
Pain in the wrists, fingers, ankles and toes
Bone fractures that are resistant to healing
Soft tissue injuries like plantar fasciitis, tennis elbow, hamstring tears and rotator cuff
Spinal injuries causing neck pain, mid and lower back pain, and sacroiliac joint pain

Following a stem cell injection, patients are typically able to return to everyday, non-high impact activities within 24 to 48 hours.

Stem cells have over 200 different growth factors with anti-inflammatory effects. A growth factor is a proteins or enzyme that can grow cartilage, repair scars, reduce inflammation, take away pain, and grow bone. Stem cell injections help the body’s own stem cells reconstruct and regenerate injured tissues and joint injuries causing pain. Instead of masking pain or providing temporary relief like cortisone and other drugs, stem cell injections can actually induce bodily healing.

Stem cells are obtained through amniotic fluid. The amniotic fluid is obtained from healthy donors during full-term cesarean births. All amniotic fluid is collected aseptically and tested prior to processing to confirm eligibility for transplantation by the Donors Suitability Standards established by the AATB and the FDA.

Amniotic stem cells are used because they are known to be more potent. As we get older, our stem cells get older. Using amniotic stem cells increases the likelihood of success while also eliminating the patient’s need to undergo a painful procedure to harvest stem cells from the pelvis bone. Typically it takes three to six weeks for patients to realize the full benefits of stem cell recruitment therapy. This is because the treatment utilizes the body’s own healing process.
